Enrollment Procedures

  1. Complete Student Enrollment Forms:

    To enter Kindergarten, a child must be 5 years old on or before September 1, 2025. To enter Transitional Kindergarten, a child must be 4 years on or before September 1, 2025.

  1. For Kindergarten or 1st Grade only, submit a completed Report of Medical Examination for School Entry (CHDP) if the student is new to the school system: download here...

    Specific dates of immunizations must be included. Return this form to the school. The form is also available from most physicians, or any school office

  1. Provide verification of age:

    Birth certificate or Passport

  1. Provide Proof of residence:

  1. Verification of Immunizations Records:

    Shots Required for Transitional Kindergarten and 7th grade State

    Law requires a verification record of immunizations be provided prior to school admittance.
